Since the Coronavirus outbreak we are adapting how we work and reducing the number of people coming into see us. This allows us to support those who need urgent or emergency care and reduce the risk of infection.
It is likely that this situation will continue for the foreseeable future, so in line with national guidance, it may be some time before our clinics are operating fully.

So, to help us continue to provide a safe and effective service now and plan for the future, we’d like to:
- Check your details to ensure we have the most up to date contact information for you as a patient on our waiting list
- ask for your consent (please complete our consent form below to be able to contact you in new ways (e.g. by email instead of post) and return to s1.dynamichealth@nhs.net.

Verification

Once you have received and returned your consent form, you will be added to out waiting list. Then you will receive an email that contains a link, which will take you to a secure website to verify your details. it's important that you do so within 7 days of receiving this email.
This information outlines the process for verifying your email address and what to expect once this stage is complete. It is important that you provide your email address and telephone numbers clearly and accurately.
By providing and verifying an email address, you will be able to receive confirmation of appointments via email; text messages can also be sent. Letters about your care (clinical letters) can also be forwarded via email so that you receive them on the day they have been sent.
You will also be able to access video appointments, as convenient as a phone call, with the added benefit of talking face to face. The software is easy to use and secure; you will have your own private video room that only we can enter. All you need is a good internet connection and a phone, tablet, laptop or computer with a camera.
However, please note you will not receive a verification email (as outlined below) if you have already verified your email with your GP.
What will the email look like?
Once we have received your consent form, we will add it to our system and send you an email from: S1.Dynamichealth (Cambridgeshire Community Services NHS Trust)
The subject heading will say Patient Record Email Verification. Please click on this link to verify your email address, within 7 days of receiving it.
What happens next?

Your referral is accepted by our service.
You are added to a waiting list and sent a consent form to complete.
You return your consent form. We send a verifuication email to the details provided.
You click on the verification email within 7 days of receiving it.
Once we receive your verification, we will contact you with an appointment when you are near the top of the waiting list.
